Transition Home

To ensure a smooth transition home, our social service director will assess and arrange the necessary services and equipment you will need to return to your home. Our therapists can also come to your home with you before discharge to help you to transition back home by assessing your specific needs at home.

We encourage you to select where you will receive your post-hospital rehabilitation services prior to entering the hospital. Our admissions coordinator can explain your Medicare benefits to you as well as assist with pre-admission to the program.
